Members of the 100 Million Healthier Lives SCALE (Spreading Community Accelerators through Learning & Evaluation) initiative engaged in intensive learning about how to unleash community transformations that can create sustained improvements in population health, well-being, and equity. This report lays out the conceptual underpinnings and empirical evidence that underlies the Community of Solutions model developed in SCALE; the companion report, “Overview of SCALE & a Community of Solutions,” details the elements of the Community of Solutions model.
