This toolkit offers a four-step framework is intended for small and medium-sized public health teams as well as larger groups of cross-sector partners and stakeholders. It helps guide teams through the iterative process of transitioning from a traditional clinical service model to one that focuses more on cross-sector, upstream interventions with their community partners.
